FAAP (Food Allergy & Anaphylaxis Care Plan) - If your child has a life-threatening allergy, please notify your school nurse, print the FAAP (Food Allergy and Anaphylaxis Emergency Care Plan) form for completion by your physician, and submit it your child's school nurse. The nurse maintains a list of students who have food allergies to share with staff on a need-to-know basis and can assist you with the necessary documents needed for school.
